U.S. Representative Jackie Walorski and three other people died in an accident in Elkhart County last month, and excessive speed was one of the contributing factors.
Zach Potts, a member of Walorski's staff, struck a car moving in the opposite direction while going roughly 80 miles per hour, according to Elkhart County police.
The deadly crash, according to police, was caused by Potts attempting to pass a slower-moving vehicle.
The report was released more than a month after the crash on August 3 that killed Walorski, 58.
The cause of death and the method of each of the four fatalities were also disclosed by officials.
The crash happened not far from the roundabout where Indiana 19 and Indiana 119 meet.
